Sounds like you're getting kicked while you're down. Actually stage four COPD is not much different from stage 3; a couple of points on the FEV1 scale makes it stage 4 instead of stage 3. So you can still do everything today that you could do yesterday and pretty much at the same speed. The heart failure is probably a bigger and new problem, At this point I hope you have a really good pulmonologist and a cardiologist and I hope they talk with each other. One of the problems with those two diagnoses is that it's often really difficult to tell what's heart and what's lungs and what might be both, so you need to be sure they're talking. That's often hard when you're really not feeling good at all. So gather your support team and make sure they know what's important to you so they can advocate with your medical team for what's best for you.
