Spacers are supposed to greatly improve the effectiveness of inhalers. I take salbutamol when having laboured breathing during exercise. I find carrying the inhaler a nuisance but it still works well enough for me. Let alone carry a spacer as well.

You can use a spacer for most inhalers that use a propeller to get the med out, but they don't work with dry powder inhalers. Many people feel that they get a better "hit" using the spacer than using the inhaler by itself.

I use a spacer on my rescue inhaler. I don't feel like I'm getting much of the medication with it.
