Try your County Social Service department. They should be able to help you figure out where to start to get help.

I am sorry a couple more ideas. If you are not on disability you need to apply that process can take a couple years. If you need to get on disability I suggest you get an attorney who specializes in helping people get disability.

An attorney will tell you everything you need to do. You can also start the disability process online. That is through Social Security and Medicaid. Social Security is the place to start that process.

YOu can also talk to your doctor about getting help in. I don;t know what insurance you have but most allow for so many hours of in home help each week. Talk to your doctor about prescribing some home health assistance for you. I am an State Tested Nursing Assistant in the state of Ohio. What we do is every thing from light house keeping to bathing. We even prepare meals. You can[t get 24/7 in home care but you could probably get enough help to help you stay in your home.

BUT pretty much start by talking to your doc.

And here in Ohio we have several agencies to assist the disabled. Google is your friend look it up and take notes.

I suggest you start with your doctor and ask if he or she can help you get into assisted living or long term care. I also suggest you do this right away. it can take time. make an appointment with your doc to do this you do not need to wait till your a ill or your check up.
