Your nurse practitioner is probably recommending one of the newer LABA/LAMA combinations (a long acting beta agonist, like serevent, combined with a long acting anticholinergic like Spiriva). They are often recommended because the research consistently shows that the two meds in combination work better than if they are take separately. By working better, they mean that people see an improvement in FEV1, in daily functioning and in things like the six minute walk done for distance, not for O2 qualification. So the NP has good reason to recommend a hew med. By the same token, you have every right to refuse it and if you do, the NP should really be quiet.

All that said, you could give it a try and see what happens. Don't if you can't afford it, and the new ones are really expensive, but see if you can't get a sample and see how it does work. You might find that it really works well for you. If you don't think it works well, then you can go right back to what you were taking.
