You ask. If your doc isn't cooperative, see if you can find a medical professional in your community who specializes in tobacco cessation programs. There are many, many aids and the professionals who understand addiction use all of them. Best practices in this area are pretty clear.

As for your family, first of all, give them a cocktail straw, tell them to hold their noses and march in place for a minute. That will give them a good idea of what your breathing is like. Then have someone wash down a wall in your house. Jeanne B is absolutely correct: all that stuff is in your lungs, essentially making holes in them, creating places where the air gets trapped and making you really short of breath. Then tell them that smoking around you could be the thing that sends you to the hospital for the last time. Most families respond pretty well to that. If yours doesn't, consider moving out. Your life could depend on it.
